The Animal Study Application (IACUC Protocol) includes several associated appendices that target specific areas of compliance oversight. Instructions for completing the application are included inline and will indicate which appendices should be completed. Following is an overview of forms and guidance documents.

Determination of Exemption for Observation-Only Studies

Complete and submit this form for observation-only studies to determine if an IACUC approved protocol is required.

Animal Study Application (IACUC Protocol)

Complete and submit this form and required appendices for any animal research to be conducted at, by, or under the auspices of the University of Mississippi; review and approval by the Institutional Animal Care and Use Committee (IACUC) is required before the research begins.
Guidance: Pain or Distress Categories

Appendix I - Experimental Disease Induction

Complete this appendix if any infectious agents will be utilized in the project or if any diseases will be induced by experimental procedure.

Appendix II - Tumor Induction

Complete this appendix if any animals will be inoculated with tumor cells.

Appendix III - Wild Caught Animals

Complete this appendix if any research will be conducted using animals procured from the wild or their native environment.

Appendix IV - Radioisotopes / Toxic Chemicals / Biological Hazards

Complete this appendix if any radiological, chemical, or biological hazardous materials or processes are utilized in the project. Please consult the Health and Safety documentation for guidelines for determining hazards.

Appendix V - Surgical Procedures

Complete this appendix if the research procedures will include any surgical or biopsy procedures.
Please note that all research personnel who perform or participate in surgery on laboratory animals in any teaching, testing, or research protocol at the University of Mississippi must receive training, demonstrate proficiency, and be certified by the Attending Veterinarian. You will need to complete and submit the Proficiency Criteria for Research Animal Surgery form to the Attending Veterinarian.

Appendix VI

This appendix has been discontinued, but its number remains reserved to prevent confusion of forms.

Appendix VII - Novel Compounds

Complete this appendix if the research project will involve new or unknown drugs, compounds, or biological extracts.
